When you use our booking form, we collect your name, phone number, email address, the reason for your visit, your preferred day and time, and any note you choose to add. We use this only to contact you and arrange your appointment.
Please don’t include medical details in the form. We’ll talk through everything with you directly.
Your clinical records from appointments are kept at the clinic and handled under Manitoba’s health privacy laws. They are separate from this website.
Information collected automatically
We use Google Analytics to understand how people use our website, such as which pages are visited, how long people stay, the type of device used, and their general location (city level). Google Analytics uses cookies to do this. This information does not identify you personally, and we never send the contents of our booking form to Google.
You can block cookies in your browser settings, or opt out of Google Analytics with Google’s browser add-on at tools.google.com/dlpage/gaoptout.
Who we share it with
We don’t sell or rent your information. We only share it with the service providers we need to run this website:
- Netlify, which hosts our website and delivers booking form messages to our email
- Google, which provides website analytics
- Microsoft, which provides our email
Some of these providers may store information outside of Canada.
How long we keep it
We keep booking requests only as long as needed to respond to you and manage your appointment, unless a longer period is required by law.

You can ask to see, correct or delete the information you’ve sent us through this website. Just contact us using the details above.
